I give my consent for Anneke and any volunteers working with Anneke to minister to me in the areas of Spiritual Guidance, Inner Healing and Deliverance.

I understand and acknowledge that Anneke is not a licensed or trained mental health professional. 

All guidance and counsel I receive will be based on Biblical principles and standards.

I understand and acknowledge that this ministry is under the direction of the Holy Spirit, and that no guarantees are made, nor can be made, with regard to my healing and or deliverance.

I state that I have voluntarily pursued this ministry for myself and that I hereby release Anneke and any volunteers working with Anneke from any and all claims of actual or implied liability that may arise now or in the future as a result of the ministry I receive.
